Licensed Practical Nurse New

Pride Health is hiring a Licensed Practical Nurse to support our client’s medical facility based in Deltona, Florida. This is a contract-to-hire opportunity and a great way to start working with a top-tier healthcare organization!

Responsibilities:

• Collaborates with providers and clinic staff to identify and manage patients with chronic disease diagnoses.
• Performs focused assessments, medication reconciliations, and motivational interviewing to support patients with chronic conditions.
• Facilitates patient education and coordinates referrals to appropriate agencies and resources for chronic care management.
• Supports the transition of patients from inpatient to outpatient settings, addressing post-discharge needs and reviewing contingency plans.
• Provides timely updates to providers and supervising RN regarding changes in patient health conditions.
• Ensures accurate documentation in the electronic medical record (EMR) and actively participates in team huddles and meetings.

Qualifications Required:

• Active and valid LPN license in good standing.
• Current Basic Life Support (BLS) certification.
• Over 2 years of experience

• Location: Deltona, Florida
• Job Type: Contract-to-hire
• Schedule: Monday to Friday – 08:00 am to 05:00 pm
• Pay Range: $ 30.00 to 31.00/hour.

*Offered pay rate will be based on education, experience, and healthcare credentials.
