Type: 13 - Week (Contract)

Shift: 8-hours Day Shift. (5X8),

Scheduled Hours: 40 Hours a Week.

Hourly Compensation: $30.00 – $31.00

What you will do:

1. Provide basic nursing care, including monitoring vital signs and administering medications.
2. Assist with patient hygiene, mobility, and daily activities.
3. Collect samples for lab tests and perform basic medical procedures.
4. Document patient progress and report changes to RNs or physicians.
5. Assisting with post-discharge needs such as prescriptions, transportation, Durable Medical Equipment (DME), appointments
6. Work with the providers and clinic staff to identify patients with chronic disease diagnoses.
7. Supports longitudinal care of the patient with stable chronic care conditions by:

• Must have an Active LPN license.
• At least two years of Experience.
